However, sometimes these rashes can be life-threatening. Any rash that appears after beginning either abacavir or any of the drugs above should prompt an immediate visit to the doctor. If he or she is not available, go to the emergency room of the nearest hospital. Although most such rashes show up within the first few weeks on a drug, they may sometimes develop later, so always pay attention. If ignored, such a rash could progress into a potentially fatal hypersensitivity reaction called Stevens-Johnson syndrome. Signs of hypersensitivity include: fever flu-like symptoms such as aches, pains, fatigue and headache with abacavir, there may be respiratory symptoms such as difficulty breathing, sore throat and a cough.

However, all things are not equal. Many factors must be considered before one can decide which type of solvent is best for your particular situation. From the regulatory point of view one must be concerned not only with the various federal and state agencies concerning emissions, but also with the FDA regarding product safety and efficacy. As various organic solvents come under fire for a variety of reasons, it will become necessary to remove those materials from formulations or to demonstrate that residues are essentially nonexistent. I believe we can anticipate the day when it will be necessary to include residual solvent statements with new drug applications and residual solvent tolerances will be established. For example, several years ago the WARF Coating Laboratory developed a solvent-based coating for an encapsulated vitamin that was intended for sue in the food industry. The product was successful and was published in the Federal Register. For all intentions and purposes, it could have been commercialized; however, the company felt uneasy about the fact that it contained a few parts per million of residual solvent. So, it was back to the laboratory to develop another coating that did not have the solvent objection. You can guess that the new coating formulation almost had to contain either an alcohol, water, or combination of both. The challenge was met, but it was the commitment of management to eliminate any potentially questionable solvents in their products that enable the technical people to pursue another, and obviously costly, development program. It is imperative that film coated products be able to demonstrate efficacy. This implies drug stability and bioavailability. Stability and bioavailability. Stability and bioavailability are greatly influenced by the selection of coating and the solvent system from which it is applied. Many drugs react in the presence of water; thus, residual moisture in the film coated product may present more of a problem than residual organic solvents. Although moisture can be eliminated by heat, some products are unstable at higher temperatures or the combination of heat and moisture humidity ; . The coating itself becomes important. Materials such as cellulose acetate phthalate and shellac can themselves be altered by the presence of moisture during extended storage. Similarly, gelatin and starch films can retrograde and change dissolution characteristics. Obtaining the desired release characteristics is not only a function of the coating material, but the coating is influenced by the presence of residual solvent. It is imperative to have specifications on residual solvent tolerances for wax containing coatings applied from solvents. A wax coating with high solvent residues will not provide the barrier properties inherent of the wax alone. There are many costs, which go into a coated product. Foremost is the cost of the coating materials themselves. If the same coating material is used, whether applied from organic or aqueous solvent, its costs will not change. Nucleoside reverse transcriptase inhibitors include zidovudine ZDV, AZT, Retrovir ; , didanosine ddI, Videx, Videx EC ; , stavudine d4T, Zerit ; , lamivudine 3TC, Epivir ; , emtricitabine FTC, EmtrivaTM ; , abacavir ABC, Ziagen ; , and zalcitabine ddC, Hivid ; . Currently all NRTIs are available as liquid formulations, except emtricitabine, and zalcitabine. Furthermore, there are several fixed dose combinations that are available as solid dosage forms: Combivir zidovudine lamivudine ; , Trizivir zidovudine lamivudine abacavir ; , Epzicom lamivudine abacavirTM ; , and Truvada emtricitabine tenofovir ; . These are difficult to use in small children due to current fixed dose formulations. Tenofovir TDF, Viread ; is the only nucleotide reverse transcriptase inhibitor available. Tenofovir differs from nucleoside reverse transcriptase inhibitors because it already contains a phosphate group therefore, the initial phosphorylation required for NRTIs activation is bypassed ; . Tenofovir is FDAapproved for use in HIV-infected individuals 18 years of age. Several clinical trials and case reports have documented renal and urinary disorders such as renal failure, Fanconi's Syndrome and nephrogenic diabetes insipidus. Furthermore, in the 903 Study, significantly greater mean percentage reductions in bone mineral density from baseline were observed versus the comparator regimen. Studies in adolescent and pediatric patients are currently underway; however, the safety and efficacy of use in pediatric patients has not been established. Delavirdine DLV, Rescriptor ; , efavirenz EFV, SustivaTM ; and nevirapine NVP, Viramune ; make up the class called non-nucleoside reverse transcriptase inhibitors. Those at greatest risk for hepatitis are women with a CD4 count 250 cells mm3 and men with a CD4 count 400 cells mm3. Current PIs with formulations appropriate for infants and children who cannot swallow pills include nelfinavir NFV, Viracept ; , ritonavir RTV, Norvir ; , lopinavir ritonavir Kaletra ; , and amprenavir AMP, Agenerase ; . Indinavir IDV, Crixivan ; , and saquinavir SQV, hard gel capsule, Invirase, and soft gel capsule, Fortovase ; , are not approved for use in pediatrics, and are not available in liquid formulations. Indinavir is recommended for consideration for children who can tolerate swallowing capsules. Atazanavir, fosamprenavir, and the newly approved tipranavir have only been approved for use in HIV-infected adults and adolescents. Dose optimization studies for children are underway for several protease inhibitors including fosamprenavir, atazanavir, and tipranavir. Enfuvirtide T-20, Fuzeon ; is the first FDA-approved fusion inhibitor and the first antiretroviral that is an injection. Enfuvirtide is an amino acid synthetic peptide that interferes with the entry of HIV into CD4 cells by inhibiting fusion of viral and cellular membranes. Enfuvirtide should be used in combination with other antiretrovirals, and has been approved in HIV-infected patients 6 years of age and older. The safety and pharmacokinetics have not been established in pediatric patients 6 years of age. Enfuvirtide should be reserved for treatment experienced individuals and not for initial therapy and vaseretic. Things are always changing in the world of HIV medicine and relevant issues for people living with HIV. Here are some recent updates we should all be familiar with: A new, single pill combination called Atripla has been approved for use in appropriate patients. It is basically three medications in 1 pill, taken once daily. None of the medicines in the pill are new, but the combination should make it easier for people to be adherent. It combines the three medicines Sustiva, Emtriva and Viread. Rumor has it there is no cost difference between one pill, vs the three separate medicines, but patients who pay a co-pay with each prescription will certainly pay a bit less for the new pill. Ask your doctor if you feel this might be a good medication for you, or if you are already on that combination. Emtriva and Viread are available in the same pill known as Truvada ; . HIV continues to move into new populations at risk. Reports in the United States show that women now represent 1 in 3 new HIV infections, and african american women account for 67% of US cases of AIDS. AIDS is the 4th leading cause of death for women ages 25 to 44. Around the world, women account for about 50% of all people living with HIV, and sexual spread has supplanted intravenous drug use as the most frequent way that HIV is transmitted for women. Finally, the Center for Disease Control CDC ; has come out with a fairly bold recommendation that everyone between the ages of 13 and 64 should be tested for HIV at least once in their life. With treatment options becoming more tolerable, convenient and effective, early identification and monitoring can help us get a better hold on preventing serious complications from this manageable disease. This recommendation may also encourage risk reduction behavioral changes, awareness and prevention efforts. Please encourage friends, family and those you care about to consider getting tested.
