A fit looking young man of 30 attended the surgery complaining of a week's history of increasing breathlessness while in bed at night. He had been registered at the practice for four years and was a rare visitor. During the previous day he had noticed some dyspnoea on exertion but he had continued working. That same night he had used pillows to prop himself up in bed and had felt more comfortable. He strode into the surgery briskly with no obvious breathlessness; he looked the picture of health. On examination the only abnormality was a little bilateral basal dullness to percussion accompanied by a decrease in vocal resonance. Even though he looked very fit I arranged for him to have an urgent chest radiograph in view of the rather bizarre history. This was performed that same morning at the local hospital, and the result was phoned through to me. The radiologist felt that there was slight bilateral raising of the diaphragm and some blurring at both bases. He suggested referring the patient to the chest clinic. As luck would have it there was a chest clinic taking place that morning, and an immediate appointment was arranged. While he waited in the x ray department the patient was informed of his appointment at the chest clinic, and he was sent over and seen there and then with his x ray film. When I rang later to find out what had happened, I was shocked to hear that he had been admitted by the chest physician with a diagnosis of bilateral diaphragmatic paresis. He was treated as an inpatient for three weeks, and after electromyography he was transferred to a specialist unit where he was found to have bilateral phrenic nerve palsies probably caused by neuralgic amyotrophy. He needed nocturnal nasal ventilation. His orthopnoea was caused by the paralysed diaphragm being pushed up into his chest by the abdominal organs when was lying down. When he stood up his respiration was maintained by his intercostals. Neuralgic amyotrophy, also known as brachial plexus neuropathy, has an incidence of 1.64 per 100 000 people. It usually presents with pain and muscle weakness with atrophy in the shoulder girdle. A rare variant affects the phrenic nerves and diaphragm. This case is unusual in that there was no pain. No specific precipitating cause has been found, but infections, inoculations, minor trauma, and surgical procedures have commonly occurred before onset. One and a half years after his admission, the patient has just recently had dramatic improvement, and he has been able to discontinue nocturnal nasal ventilation. He is still being followed up. It would have been easy to dismiss his initial symptoms and minimal signs, and it was just lucky that everything fell into place for him on that morning in July. Few epidemiological studies have been conducted specifically in this condition.
