Cute renal failure ARF ; is a serious complication after cardiac surgery resulting in significant in-hospital and long-term morbidity and mortality, as well as prolonged hospital stay.1, 2 The risk of ARF ranges from 5% to 31%35 and is even greater in high-risk populations or when more inclusive definitions are used.6 When it is severe enough to require renal replacement therapy, the mortality rate is close to 60%. Fenoldopam mesylate, a benzazepine derivative, is the first selective dopamine-1 receptor agonist that has been approved for clinical use hypertensive urgencies and emergencies ; . The selective dopaminergic action of fenoldopam appears to improve renal performance in situations of reduced renal blood flow such as severe hypertension, 7, 8 hypertensive patients with preexisting impaired renal function, 9 and ventilation with positive end-expiratory pressure.10 These effects are mediated by increased renal blood flow to both cortex and medullary regions. Fenoldopam may have some utility during cardiopulmonary bypass CPB ; because renal ischemia is thought to contribute to postbypass renal dysfunction. Data on fenoldopam clinical efficacy are sparse, and there is still virtually no definitive evidence of efficacy, particularly and cefuroxime.

Genital ulcers facilitate the spread of HIV more than other STIs RTIs. Chancroid, genital herpes and syphilis are common in regions where HIV prevalence is high, and control of these infections is an important component of HIV prevention. The presence of HIV infection may also change the presentation of genital ulcers making their diagnosis more difficult. Lesions of primary and secondary syphilis may be atypical. Chancroid lesions may be more extensive, and rapidly aggressive lesions have been noted. This reinforces the need for early treatment, especially in HIV-infected individuals. Treatment of genital ulcers is the same for HIV-positive and HIV-negative patients. All patients should be seen one week after starting treatment, and treatment should be continued if significant improvement is not apparent. Herpes simplex lesions may present as persistent multiple ulcers that require medical attention. Antiviral treatment may reduce symptoms. The nature and purpose of treatment should be properly explained to the patient in order to avoid false expectations of cure.

Although radiation therapy is to our knowledge the single most effective agent in the treatment of Hodgkin disease, the role of consolidation irradiation adjuvant to bone marrow and PBSC transplantation remains unclear. While retrospective analyses 1014 ; have shown a reduction in the risk of local tumor relapse with the addition of local radiation therapy, to our knowledge, results of randomized studies have not been reported, and the routine use of adjuvant radiation therapy remains controversial. The majority of relapses are at sites of previous disease 11 ; , and an argument can be made for the use of adjuvant radiation therapy if the accompanying toxic reaction is acceptable. The effect on overall toxic reaction has been difficult to discern, as findings of early studies 4, 14, 1618 ; of both allogenic and autologous bone marrow transplantations showed high treatment-related mortality and morbidity, but local peritransplantation radiation therapy has been implicated as a contributing factor to mortality in up to 7%14% 10, 12, ; of patients. With advances in supportive care and the shift to autologous PBSC support, overall mortality and morbidity have decreased 6, 7, 19, ; , and this provides a more optimal scenario to assess the effects of adjuvant irradiation. In the present series, the toxicity of consolidation radiation therapy did not contribute to any deaths. The main purpose of this study was to assess the hematologic feasibility of routine, adjuvant external-beam irradiation. Few studies have focused on this topic, and to our knowledge this is the first study in which the hematologic toxic reaction to high-dose chemotherapy with PBSC support in patients with Hodgkin disease who do and in those who do not undergo irradiation has been evaluated. The file card also contained misleading claims pertaining to the effect of Exelon on behavior. In particular, a graph presented the percentage of patients Exelon vs. placebo ; with symptoms at baseline improving on the individual components of the Effect on Behavioral Pathology in Alzheimer's Disease BEHAVE-AD ; scale. Other claims in the file card suggested that Exelon can help manage some behavioral aspects of Alzheimer's Disease, which has not been demonstrated by substantial evidence or substantial clinical experience. These claims were misleading for several reasons: First, highlighting a non-significant finding is inherently misleading as the observation may be no more than a chance observation. Second, the BEHAVE-AD scale was not a designated primary or secondary efficacy measure in any of the three pooled studies cited in the piece. Finally, the referenced retrospective pooled analysis was a subset analysis, which was subject to bias and is non-credible. The disclaimer that statistical significance was not shown is not sufficient to correct the misleading impression made by the claim. As technology transforms how information is stored, accessed, manipulated and disseminated, the concept of information technology IT ; changes as well. These changes are significant and dramatic, affecting the nature of learning and the creation of knowledge itself. Medical education, defined in a broad sense as teaching and administrative activities pertaining to the training of medical and para-medical staff as well as public health education, is expected to benefit tremendously from the growth of IT. Under the leadership of the Prime Minister of Malaysia, Datuk Seri Dr. Mahathir Mohd, Malaysia aspires to be the regional centre of excellence in education. By this, the government expects a significant increase in the quantity of students as well as the quality of education in the country within the next 5 years. Normally, this aspiration will necessitate huge expenditure in the form of building new infrastructure and increased manpower intake. The government, however, through its "Vision 2020" program, believes that the former may be achieved without the high expenditure of the latter. The `ticket' to this success or at least a major part of it ; is the acquisition of IT; in tandem with the transformation of the nation from an agro-based to a knowledge-based society in order to achieve and maintain economic success, and thus a `developed nation' status by the year 2020. The objective of this talk is to draw attention to the recent advances in computer hardware, software and telecommunications and the convergence of these technologies which have produced such phenomenal successes as the InterNet, IntraNet, interactive multimedia IM ; and "telepresence". Today, Malaysians are indeed blessed with good management and leadership foresight as seen by the modern telecommunication infrastructure already in existence, including the KL Tower, Petronas Twin Towers, MESAT I & II satellites, Nation-wide fiber optic network and the soon to be completed mega projects, such as Cyberjaya, Multimedia Super Corridor MSC ; and her much heralded seven flagship application programs, which include "Telemedicine" and "Smart Schools". These mean that there is now a multiplicity of tools which offer the potential for significant time savings in communication and improved quality of medical education through innovative use of IT in course-ware and curriculum designs. Case in point, a number of demonstrations will be made to showcase "IT in medical education" achievements in Malaysia. Considering the above, the task of using IT to achieve excellence in medical education is now in the hands of educators and administrators. Dare we take the challenge?.
