Study No.: 550 Title: A Randomised, Double-Blind, Double-Dummy, Multicentre, Parallel Group Study to Assess the Efficacy and Safety of Oral Augmentin SR, Two Tablets Equal to 2000 125mg, Twice Daily for 10 Days Versus Lvofloxacin Levaquin ; 500mg Once Daily for 10 Days in the Treatment of Adults with Acute Bacterial Sinusitis Infections. Rationale: The purpose of this study was to demonstrate that oral Augmentin SR tablets pharmacokinetically enhanced amoxicillin clavulanate ; 2000 125mg twice daily bid ; for 10 days is at least as good as oral levofloxacin 500mg once daily od ; for 10 days in the treatment of acute bacterial sinusitis ABS ; . Phase: III Study Period: 15 November 1999 to 14 February 2000. Study Design: Randomised, double-blind, double-dummy, multicenter, parallel group study. Subjects were instructed to attend the clinic at screening Visit 1, day 0 ; , on-therapy Visit 2, day 3-5 ; , end of therapy Visit 3, day 12-14 ; and follow up Visit 4, day 17-24 ; . Centres: A total of 62 centres in France 19 ; , Germany 14 ; and US 29 ; . Data from 2 US centres were removed as requested by the Food and Drug Administration FDA ; . Indication: Acute bacterial sinusitis. Treatment: Subjects were randomized to one of two treatment groups, in a 1: ratio Treatment group A: 10 days of oral treatment with amoxicillin clavulanate SR 2000 125mg bid plus placebo to levofloxacin 500mg od or Treatment group B: 10 days of oral treatment with levofloxacin 500mg od, plus placebo to amoxicillin clavulanate SR 2000 125mg bid. Objectives: The primary objective was to demonstrate that the clinical efficacy of amoxicillin clavulanate 2000 125mg twice daily for 10 days was at least as effective as that of levofloxacin 500mg once daily for 10 days in the treatment of ABS. The secondary objective was to evaluate the bacteriological efficacy and safety of amoxicillin clavulanate SR 2000 125mg twice daily for 10 days compared with levofloxacin 500 mg once daily for 10 days in patients with ABS. Primary Outcome Efficacy Variable: The combined clinical and radiological response success, failure or unable to determine ; at follow up Visit 4 ; . Clinical success was defined as sufficient resolution of signs and symptoms of ABS such that no additional antibacterial therapy for ABS was indicated. Clinical failure at follow up was defined as reappearance or deterioration of signs and symptoms of ABS following clinical success at end of therapy. If a patient was deemed to be a clinical failure at any visit, this outcome was carried forward to all further visits. Radiological outcome of "improved" was defined as improvement or resolution of radiological signs of ABS. Radiological outcome of "unchanged" was defined as no improvement in the baseline radiological signs of ABS. Radiological outcome of "worse" was defined as a worsening of the baseline radiological signs of ABS, or the appearance of new radiological signs of ABS. "Unable to determine" was defined as a valid assessment of radiological outcome could not be made. Combined clinical and radiological response at follow up was defined as success when there was a clinical outcome of success and the radiological outcome was improved or unchanged. Combined clinical and radiological response at follow up was defined as failure when there was a clinical outcome of failure at end of therapy, or clinical outcome at follow up was recurrence, and or a radiological outcome was worse at follow up. Combined clinical and radiological response at follow up was defined as unable to determine when either the patient's clinical outcome at end of therapy or follow up was unable to determine and radiological outcome at follow up was improved, unchanged, unable to determine, or unknown; or the clinical response at follow up was success and the radiological outcome was unable to determine or unknown. Secondary Outcome Efficacy Variable s ; : Clinical response success or failure ; at follow up Visit 4 ; Clinical response success or failure ; at end of therapy Visit 3 ; . Bacteriological response success or failure ; at follow up Visit 4 ; . Bacteriological response success or failure ; at end of therapy Visit 3 ; . Bacteriological success was defined as the eradication or, in the absence of an evaluable repeat culture sample, clinical evidence of eradication of all initial screening pathogens without superinfection or new infection. Bacteriological failure was defined as the persistence or recurrence of an initial screening pathogen, or the presence of a new pathogen in a repeat culture sample. For patients with no repeat culture sample available, bacteriological failure was presumed if clinical signs and symptoms persisted to a degree that necessitated further antibacterial therapy for the.
Impact on the health infrastructure and systems The chronic illness associated with HIV AIDS has led to a reduction in the quality of health service provision and an increased mortality. 70% of hospital bed` occupancy is attributed to HIV AIDS, this has increased pressure and demand on the health infrastructure. The hospital manpower has been incapacitated because nurses and doctors are also affected by the pandemic. Impact of HIV AIDS on the structures and the education system There is a low quality education services, low school enrolment because, teachers are Sick and dying from HIV AIDS, there is absenteeism from work, a decrease in the teaching staff. Sick parents and the many AIDS orphans mean that Impact of HIV AIDS on the economic systems HIV AIDS affects mainly people in the economically productive years. In Burundi it is the head of the family who are poor, not insured and without social security support that are mostly affected. HIV AIDS leads to increase of costs of health care for households, companies, social security and state. The HIV AIDS impact on the people's productive capacity has made direct impact on the agricultural sector, formal and informal private sector, the public at large. Demographic impact of HIV AIDS The UNAIDS report projects that , life expectancy at birth in Burundi will go down from 46 years of 1997 to 39 years in 2010. Impact of HIV AIDS on other social aspects HIV AIDS affects has severely affected families, death of the bread winner or one of the parents, has a consequence of loss of income and deprivation of survivors of the emotional and material support. Orphans are a huge problem and on the other hand, women risk being deserted by husbands when HIV AIDS positive status is discovered.

Savarese, 34 Washington Street, Wellesley, MA 02481, United States] - CANCER TREAT. REV. 2003 29 6 ; summ in ENGL Goals of the work. Malignancy produces a state of physiologic stress that is characterized by a relative deficiency of glutamine, a condition that is further exacerbated by the effects of cancer treatment. Glutamine deficiency may impact on normal tissue tolerance to antitumor treatment, and may lead to dose reductions and compromised treatment outcome, Providing supplemental glutamine during cancer treatment has the potential to abrogate treatmentrelated toxicity. We reviewed the available data on the use of glutamine to decrease the incidence and severity of adverse effects due to chemotherapy and or radiation in cancer patients. Methods. We performed a search of the MEDLINE database during the time period 1980-2003, and reviewed the English language literature of both human and animal studies pertaining to the use of glutamine in subjects with cancer. We also manually searched the bibliographies of published articles for relevant references. Main results. The available evidence suggests that glutamine supplementation may decrease the incidence and or severity of chemotherapy-associated mucositis, irinotecan-associated diarrhea, paclitaxel-induced neuropathy, hepatic veno-occlusive disease in the setting of high dose chemotherapy and stem cell transplantation, and the cardiotoxicity that accompanies anthracycline use. Oral glutamine supplementation may enhance the therapeutic index by protecting normal tissues from, and sensitizing tumor cells to chemotherapy and radiation-related injury. Conclusions. The role of glutamine in the prevention of chemotherapy and radiation-induced toxicity is evolving. Glutamine supplementation is inexpensive and it may reduce the incidence of gastrointestinal, neurologic, and possibly cardiac complications of cancer therapy. Further studies, particularly placebo-controlled phase III trials, are needed to define its role in chemotherapy-induced toxicity. 2003 Elsevier Ltd. All rights reserved. 670. ParE, subunit mutations 20, 3441 ; . In summary, fluoroquinolone resistance is mediated by changes in DNA gyrase or topoisomerase IV intracellular enzymes necessary for bacterial replication. Each of these vital intracellular enzymes is composed of two subunits. The two primary determinants of quinolone resistance are the gyrA subunit of DNA gyrase and the parC subunit of topoisomerase IV. A mutation in either of these subunits results in an increase in S. pneumoniae MICs to quinolones. High level resistance requires a two-step mutation involving subunits from both DNA gyrase and topoisomerase IV. Multi-drug resistant S. pneumoniae MDRSP ; Penicillin resistant strains of S. pneumoniae are also frequently resistant to macrolides and TMPSMX. If pneumococci are resistant to three different antimicrobials, they are said to be MDRSP. Because of the difference in use of macrolides and TMPSMX, the likelihood of MDRSP strains varies considerably in different geographical areas. It is an important therapeutic consideration in many areas. MDRSP strains are usually only sensitive to respiratory quinolones levofloxacih and miconazole.

They're also used in the latest rejuvenation treatments, which are designed to turn up the clarity and radiance level in complexions that are starting to show their age. The fact that they also work to improve breakout-prone skin is a reminder that skin inflammation, which triggers free-radical damage, takes away clarity and leaves a dull look at any age. In fact, there's growing evidence that acne inflammation causes the skin to age at a faster rate than otherwise. ; So with the Nu Skin Clear ActionTM Acne Medication System, you're not only helping your skin to "act its age" by keeping blemishes away, but you're simultaneously coaxing it to look its most vital, too.

In this case, AstraZeneca's own evidence confirmed that none of the required conditions were actually met. They commissioned the insert, they paid the authors, they paid The Journal to distribute it, they reviewed the document and they provided information for inclusion albeit at the request of the authors ; .Taken together, the circumstances were such that it became impossible for AstraZeneca to argue that the insert was independently produced as far as the Code of Practice was concerned. This case has resulted in one of the highest number of upheld complaints ever recorded for a single item, 27 in all: Three breaches of Clause 2 for bringing the industry into disrepute ; Five breaches of Clause 9.1 for not maintaining high standards ; Five breaches of Clause 10.1 for conducting disguised promotion ; One breach of Clause 7.10 for failing to encourage the rationale use of a medicine ; Three breaches of Clause 4.1 for not including prescribing information in the insert ; Eight breaches of Clauses 7.2 for making various misleading claims ; Two breaches of Clause 7.4 for making claims that were not capable of substantiation ; . Some readers might be puzzled that the same breaches were not consistently applied to all five complaints. This is because of the way the code is applied. Essentially, each complaint is considered individually on the alleged breaches raised by that complainant. Therefore if the complainant does not list a particular allegation it is not considered for that specific case. So, three of the complainants raised issues relating to patient safety.

Company in South Africa. China and Japan have built small demonstration reactors. NRC commissioner Jeffrey S. Merrifield, speaking on Oct. 2, 2006 at the Johannesburg conference on High Temperature Reactor Technology, said that deployment of a high-temperature-gas nuclear reactor "is likely more than ten to fifteen years away." But he also took on the hydrogen-manufacturing component, noting that many people in the U.S. equate hydrogen use "with the Hindenburg dirigible disaster dating back to 1937." And while advances in hydrogen safety have been made, he says predictable questions on having these technologies side by side will be asked when nuclear power is tied in with hydrogen production. nuclear plants in the next 20 years. Klein points out that the NRC is working to complete or update the design certifications on advanced reactor designs such as the ESBWR Economic Simplified Boiling Water Reactor ; and the AP1000. But most important is that the nuclear industry has announced intent to submit 19 applications for combined operating licenses COLs--part of the streamlined application process in the EPACT 1992 ; covering 27 new reactors and Klein says more are in the works. He also notes the enormous work ahead associated with reviewing the license application for the Yucca Mountain nuclear waste repository, and licensing applications for the MOX mixed-oxide fuel ; facility for conversion of weapons grade plutonium to fuel usable in commercial nuclear plants, and the American centrifuge facility where USEC U.S. Enrichment Corp. ; demonstrates its next-generation uranium enrichment technology. But Klein's predictions of revitalization are helped along by Title 17 of EPACT 2005 that authorize the federal government to provide loan guarantees for up to 80% of the cost of innovative technologies such as nuclear and clean coal that will "avoid, reduce or sequester greenhouse gases." But while DOE guidelines allow for loan guarantees for projects, the Nuclear Energy Institute NEI ; asked Congress to give DOE more guidance.

Abstract 1705 DEVELOPING A SYMPTOM AND HRQL QUESTIONNAIRE FOR UTERINE FIBROIDS Karin Coyne, Medha Sasane, Sheila Walsh, James Spies, Center for Health Outcomes Research, MEDTAP International, Inc., Bethesda, MD Uterine fibroids UF ; affect approximately 20 - 25% of the female population. While many women with UF are asymptomatic, those who do experience UF symptoms are greatly impacted by their symptoms. Treatments for alleviation of UF symptoms range from medical therapy to hysterectomy; new treatments are designed to reduce symptoms where retaining the ability to bear children. There are no disease-specific measures to evaluate treatment outcomes for UF. As such, we are developing a self-administered, patient based symptom and health-related quality of life HRQL ; questionnaire specific to UF. After a thorough literature review, we conducted two focus groups n 17 ; to explore the range and depth of the uterine fibroid symptom experience including its impact on HRQL. Phrases and words that the participants used to describe the symptom experience were also collected. Using content analysis, we identified 18 items for inclusion in the symptom scale, grouped in 3 dimensions: menorrhagia, bulk symptoms, and pain; 54 items were identified for inclusion in the HRQL scale grouped in 8 dimensions: fatigue, sleep, self-image, mood disturbances, fear of embarrassment, interference with daily activities physical, social, work ; , relationships with family and friends, and sexual functioning. Participants were also asked to complete the SF-36 as a generic indicator of HRQL; all subscale scores were substantially lower 5 points ; than population norms indicating poorer HRQL in all domains in this patient cohort. Focus group participants were asked to provide feedback on the preliminary questionnaire 76% response rate ; . Respondents rated the item content as consistent with the focus group discussions and pertinent to women with UF. Content validity was also assessed through clinical expert review n 5 ; and cognitive debriefing interviews in women with UF n 5 ; Results indicate the questionnaire is content valid, feasible, and relevant to women with UF.

In the fall of 2000, I found myself at the Miami airport waiting to board a flight to DC for the NIH Consensus Conference on Early Breast Cancer. This was my pre-cell phone era, and my beeper went off alerting me to a call I had been anxiously anticipating for weeks. Greg McIntosh, from the market research firm ReedHaldyMcIntosh, wished to speak with me. I scrambled for a pay phone to find out about the initial report from the first formal external, independent study of our breast cancer audio series. Greg told me that the initial five interviews with medical oncologists went flawlessly, and based on this, he was confident that his team would be able to recruit and study the remaining 145 randomly selected United States-based physicians in the next few weeks. For some inexplicable reason, this situation was reminiscent of my clinical research years as a member of the University of Miami faculty, when patients would return after their first course of an experimental therapy and I would anxiously pull out x-rays remember those? ; to assess for response. "Greg, how many of these first five docs listen to our program?" I held my breath waiting for the answer. Greg paused for a moment, checking his notes, and said, "Let's see.1, 2, 3, 4.four of the five." My prolonged exhalation lasted the entire five-hour air and land journey to the Holiday Inn in Bethesda. After years of wondering whether our work was having an impact, the answer was about to emerge. Third-party continuing medical education programs supported by grants from pharmaceutical companies and distributed without charge to physicians are rarely evaluated independently. Minimal scientifically validated information exists to indicate how often physicians utilize these resources. Programs are generally considered successful if used by five to 10 percent of recipients, and as we all know, a plethora of such educational publications regularly inundate oncologists' mailboxes. Although it was clear that some oncologists listened to our programs, I was concerned that the "ratings" would be minuscule and that perhaps our company would go out of business. Needless to say, the decision to conduct this study was one of the greatest risks of my totally non-MBA-like business career. In one sense, the survey would differentiate our work from many other CME groups because it was an independent review. The core of our group includes.
