Behavioral health pharmaceuticals are a substantial portion of state pharmacy costs in Medicaid. As beneficiaries that are dually eligible for Medicaid and Medicare begin to receive the Medicare drug benefit, the portion of state pharmacy costs attributable to AAPs, second generation antidepressants, and other behavioral health pharmaceuticals is expected to rise. With this increase, states may pay even more attention to managing behavioral health pharmaceuticals in Medicaid. In the retina, astrocytes and Mller cells become reactive, expressing high levels of GFAP, becoming hypertrophied and proliferating [2, 3, 7, 9]. Elsewhere in the central nervous system, astrocytes are the main participants in the gliotic response [28, 29]. Following an injury to the retina, we have confirmed the dramatic increase in the levels of GFAP and have demonstrated a significant upregulation in the levels of TAPA. This increase in TAPA is reminiscent of the upregulation observed following a cortical stab wound [11, 30]. The elevation of TAPA following retinal injury is associated with the reactive response of glial cells, similar to that seen in injury-induced glial scar formation in the brain [30]. In the present study we demonstrated a 1.7-fold upregulation of a glial membrane protein that is associated with the regulation of mitotic activity and stabilization of cellular contacts [11]. This may appear to be a minimal increase in the levels of TAPA; however if this protein is only expressed in retinal glia this upregulation may represent a substantial increase per glial cell. In the rat retina there are approximately 31 neurons per Mller glial cell [31]. Thus, the 1.7 increase in TAPA relative to total retinal proteins would represent an approximate 20-fold increase in the levels of TAPA per Mller glial cell. These findings provide a key insight into the molecular mechanisms regulating the proliferative response of non-neuronal cells in the injured retina by raising the possibility that TAPA may be a molecular switch through which reactive gliosis and retinal scarring can be controlled. In the area of medicinal chemistry artificial receptors can be used, e.g., as mediators for drug transport carriers, protectors, dissolution mediators ; [1] or as model systems for more complicated systems such as proteins [2]. Our method focuses on the fast and reliable prediction of the structure of complexes between ligands and artificial receptors, which is a necessary prerequisite for their molecular design. The approach is based on the protein-ligand docking program FlexX [3]. In order to handle the full conformational flexibility of both molecules a new docking strategy is applied: First, both the ligand and the receptor are decomposed into fragments by cutting at each acyclic bond. For both molecules a set of fragments with many directional interactions is selected. Second, each conformation of each of these fragments of the ligand is docked to each of the conformers of the receptors fragments. The resulting complexes are scored by an empirical scoring function [4].
