Disc at 2 years after trabeculectomy using the Heidelberg Retina Tomograph HRT ; . To determine the factors that most influenced change of optic disc topography. To investigate the evolution of change within the optic disc after trabeculectomy in patients with early and advanced glaucomatous neuropathy. MATERIAL AND METHODS 116 consecutive patients undergoing routine trabeculectomy at the Wroclaw Medical University, were recruited into the study. Eyes were imaged with HRT preoperatively, and at 2 weeks, 6 months, 12 months and 2 years after surgery. All the 12 global parameters of HRT were investigated for change. Patients were also divided into 2 groups: early and advanced glaucoma. HRT analysis for change in these groups was done comparatively as well as segmental analysis in 6 sectors of the optic disc. Octopus visual field test has been done before trabeculectomy and 6, 12 and 24 months after surgery in all patients. RESULTS The images of 85 patients were analysed. Intraocular pressure reduced from 29.25 SD 8.1 ; mmHg, at the time of preoperative imaging to 9.67 SD 4.0 ; mmHg at 2 weeks, 13.71 SD 3.3 ; at 6 months, 13.94 SD 3.0 ; mm Hg at year, and 14.58 SD 2.9 ; mmHg at 2 years after trabeculectomy. 2 weeks after trabeculectomy 10 HRT parameters were found to be statistically significant: cup area CA ; , c d area ratio, cup volume CV ; , rim area RA ; , rim volume RV ; , mean cup depth, cup shape measure CSM ; , height variation contour HVC ; , and mean RNFL thickness and RNFL cross sectional area. 6 months after surgery 5 parameters CA, CV, RA, RV, mean cup depth ; were statistically changed. Mean cup depth reduced significantly by month 12 and month 24. In the group of early glaucomatous patients increase in rim volume, decrease in cup volume and more negative values of CSM were present at all time points after surgery. In the group with advanced glaucoma improvement of the HRT parameters was shown only 2 weeks after surgery. CONCLUSIONS Reversal of disc cupping in adults at 2 years after trabeculectomy is available. The factor most influencing change is reduction of intraocular pressure. In the group of early glaucomatous patients, reduction of IOP by surgery results in the reversal of disc cupping, whereas in patients with advanced glaucoma stabilises the optic disc appearance and amoxicillin.

Sixteen of the 127 total reports involved interactions between cisapride and other drugs, including carbamazepine, clarithromycin, diltiazem, erythromycin, fluconazole, itraconazole, metronidazole, nefazodone, omeprazole, paroxetine, tacrolimus, warfarin and zafirlukast; 9 of these reports involved heart rate and rhythm disorders. As a result of postmarketing surveillance, important changes were introduced in September 1999 to the product monograph, prescribing information and other labelling material. A cautionary statement was added advising against concomitant use of grapefruit juice with cisapride, as it increases the bioavailability of cisapride.4 Grapefruit juice is a CYP3A4 enzyme inhibitor and acts predominantly by impeding presystemic cisapride metabolism, mediated by CYP3A4 enzymes in the small bowel, thus raising plasma concentrations of cisapride.5 Studies have confirmed that inhibition by grapefruit juice of CYP3A4 enzymes may affect the absorption of cisapride for up to 24 hours; grapefruit juice does not inhibit hepatic drug elimination.57 Moreover, a significant amount of interindividual variation prevails.6 The CADRMP has not received any reports of interactions between cisapride and grapefruit. This may be due to the fact that drugfood interactions frequently go undetected; therefore, health professionals must be vigilant in recognizing and reporting them. Other revisions to the product monograph now state that the use of cisapride in patients with known congenital or familial long QT syndrome and clinically significant bradycardia is contraindicated, as is the use of concomitant medications known to prolong the QT interval.4 These include, but are not limited to, certain anti-arrhythmics e.g., quinidine, procainamide, disopyramide, amiodarone and sotalol ; , antidepressants e.g., amitriptyline, maprotiline ; , antipsychotics e.g., certain phenothiazines and pimozide ; , antihistamines e.g., astemizole and terfenadine ; and halofantrine.4 Furthermore, as is already indicated in the product monograph, the use of cisapride is contraindicated in patients taking CYP3A4inhibiting drugs, including macrolide antibiotics e.g., erythromycin and clarithromycin ; , antifungals e.g., fluconazole, itraconazole, ketoconazole ; , HIV protease inhibitors e.g., ritonavir, indinavir ; and antidepressants e.g., nefazodone ; .
